LATERAL CAPSULAR SIGN ----- CASE CONTRIBUTED BY PROF DR.M.G.K MURTHY ,SENIOR CONSULTANT RADIOLOGIST , VIJAYA GROUP OF DIAGNOSTICS.

21 yr old female with skiing injury and pain shows subtle, ill defined opacity in relation to the lat tibial condyle with no definite donor site and asymmetrical tibial plateau- segonds fracture is possible and MR is needed for ruling out associated injures including ACL tear




Points of interest



Also called “lateral capsular sign”



It refers to Middle lateral capsular ligament injury



Lateral capsular ligament has anterior(from patella or patellar tendon to ilio- tibial band), middle (between ilio-tibial band and fibular collateral ligament), and posterior section(fibular collateral ligament to posterior cruciate ligament)



Skiing, Basket ball and baseball games associated with this



Associated bony injuries include tibial eminence (ACL att), fibular head,etc



Usually associated with ACL Tear(100%)



Tunnel view is better if AP projection is doubtful



Gerdy tubercle of tibia for insertion of Iliotibial band, along with fibular collateral ligament calcification are differential



Usually parallel to tibia long axis and vertical with about 2-10 mm below the tibial plateau and is few mm in size
